I think hearty appetite and underweight/very underweight makes sense. As far as your weight is concerned, that is very easy to resolve. When you select it at character creation, it gives -2 fitness, but any time your fitness is 2-4 for any reason it shows up on your info screen. It has no effect if you don't start with it; it's just an indicator of low fitness. It goes away when you reach 5 fitness.To lose weight in Project Zomboid, you will need to eat low calory food items and burn calories through exercising regularly. You can start exercising by going to the health tab by pressing the heart icon at the top left of your screen. Underweight, you just eat fatty foods anytime you can. I personally choose underweight so I'm not slow.
